This Special Needs Plan is for people who have Medicare Parts A & B and Medicaid. It offers more benefits and features than original Medicare. Plus, your Medicaid benefits stay the same. Along with extra benefits, the plan covers hospitals, doctors and drugs.

Medicaid

If you're eligible for Medicaid, you may also be eligible for managed long-term care sponsored by the State of New York. UnitedHealthcare Personal Assist™ from UnitedHealthcare Community Plan, provides a wide range of long-term care support and services. You can choose your own specialists. And you get in-home services to help with daily activities.

NY Medicaid Advantage (UnitedHealthcare Dual Advantage) offers an excellent opportunity for people who are eligible for Medicaid and Medicare to enroll in UnitedHealthcare for most of their Medicare and Medicaid benefits - plus great additional benefits like no monthly premium and free transportation to medical appointments.

UnitedHealthcare Wellness4Me

Medicaid


The Wellness4me Plan is offered through UnitedHealthcare Community Plan. Wellness4Me is a Health and Recovery Plan, or HARP, approved by New York State. HARPs are a new kind of plan that provide Medicaid members with their health care, plus care for behavioral health, including Serious Mental Illness and Substance Use Disorders.

This plan is for New York residents age 21 and who qualify for Medicaid coverage.

Essential Plan

The Essential Plan is offered through UnitedHealthcare Community Plan. It's for lower income individuals in New York who meet the income or citizenship status requirements.

* Vision and dental benefits are available through purchase of Essential Plus plans or as a standard benefit in Essential Plan 3 and 4 plans; these benefits are not available for Essential Plan 1 and 2 Standard Plans. Cost sharing will apply for those in Essential Plan Plus plans.

Child Health Plus

Medicaid


The New York State Child Health Plus program from UnitedHealthcare Community Plan is for children up to age 19. Members must be residents of New York and are not eligible for Medicaid or enrolled in or access to NYSHIP benefits. You can choose your child’s doctor. And your child gets many services to stay healthy.
