The New York State Medicaid Managed Care Plan is offered through UnitedHealthcare Community Plan. It’s for New York State residents who meet the income or disability requirements.

Make sure your primary care provider (PCP) is correct. The PCP listed on your member ID card is your assigned PCP. If you change your PCP, please contact Member Services. We’ll send you a new card with the updated PCP information.

Medicaid: More for you in 2024

The New York State Medicaid Managed Care Plan is offered through UnitedHealthcare Community Plan. It’s for New York State residents who meet the income or disability requirements.

Covered doctor visits:

  • Annual wellness visits
  • Well-child visits (HealthCheck)
  • Primary care provider (PCP) visits
  • Specialists visits

Standard services:

  • Immunizations

Dual Special Needs plans in 2024

New Yorkers on a UnitedHealthcare Dual Special Needs plan (or dual health plan) get many extra benefits beyond those you get with either Original Medicare or Medicaid.

That includes:

  • A monthly credit to buy healthy food and OTC products or pay utility bills
  • $0 copay on covered generic and brand-name prescriptions including home delivery
  • Meals delivered to your door after every inpatient hospital stay

Essential Plans in 2024

You may qualify for low- or no-cost health care. The Essential Plan offered through NY State of Health by UnitedHealthcare® Community Plan is for lower-income individuals in New York, ages 19–64, who don’t qualify for Medicaid or Child Health Plus.

What does it include?

  • Dental care
  • Routine eye exams, including glasses
  • Hearing exams and hearing aids
  • Prescription drugs
  • Vaccines, screenings and more

Healthy First Steps®

The Healthy First Steps team is here for you. Our program will help you take the right steps to keep you and your baby healthy.

We’ll help you:

  • Choose a pregnancy provider and a pediatrician (child’s doctor)
  • Schedule visits and exams and arrange rides to your visits
  • Get supplies, including breast pumps for nursing moms
  • Connect with community resources such as Women, Infants and Children (WIC) services
  • Childbirth education reimbursement*

Dual Special Needs plan transportation

If you live in Nassau or Suffolk County, you can get rides to and from doctor visits by calling ModivCare Solutions at 1-844-678-1103, TTY 711. If you live in a different county, call 1-844-666-6270, TTY 711. Your local Medical Answering Services will provide transportation.

The service area includes Albany, Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Orange, Orleans, Oswego, Putnam, Rensselaer, Rockland, Saratoga, Schenectady, Schoharie, Schuyler, Seneca, Steuben, Sullivan, Tioga, Ulster, Warren, Washington, Wayne, Westchester, Wyoming and Yates counties.
