This Special Needs Plan is for people who have Medicare Parts A & B and Medicaid. It offers more benefits and features than original Medicare. Plus, your Medicaid benefits stay the same. Along with extra benefits, the plan covers hospitals, doctors and drugs.

Adams, Buffalo, Burt, Cass, Dodge, Douglas, Gage, Hall, Lancaster, Madison, Otoe, Saline, Sarpy, Saunders, Seward, and Washington.

    $1,500 toward dental services.

  • Health Products Card

    Up to $560 loaded onto your card to buy health products you may need.

  • Vision Coverage

    Annual exam and $200 credit every year for eyewear.

UnitedHealthcare Community Plan - Heritage Health is for Nebraska children, families, pregnant women and adults who meet income requirements. We also offer extra support to adults with special health care needs or a disability. Members can still choose their own doctor.
