This dual health plan is for people who qualify for both Medicaid and Medicare Parts A & B (Original Medicare).

This is a Health Maintenance Organization (HMO) plan. With an HMO plan, you must use network providers to get your medical care and services.

Dental services for UnitedHealthcare Community Plan members 21 years of age and older will be available starting January 1, 2018. 

We offer health coverage to beneficiaries of Maryland’s HealthChoice program in all counties of the state.  Enrollment is open year-round. We are currently accepting new enrollees in the following counties: Baltimore City, Anne Arundel, Baltimore, Carroll, Cecil, Charles, Harford, Howard. Montgomery, Prince George’s and St. Mary’s counties.
