UnitedHealthcare Community Plan is honored to have served Iowans in the IA Health Link and hawk-i programs. Unfortunately we will no longer be participating in these programs.  We will continue to serve our Dual Complete members, and we remain committed to ensuring a smooth and seamless transition for all of our existing IA Health Link and hawk-i members.   • Care for Conditions

    If you or a family member has special health care needs, you can count on us. Our plan will provide the needed extra care and services. Benefits include:





    • Mental Health. Counseling and other treatments are covered.
    • Diabetic Support. Medicine needles and wipes for managing diabetes.
    • Asthma and COPD Support. Exams, testing and supplies are covered.
    • Care Management. If you qualify, get personal help managing health conditions.
    • Disease Management. Focused care for those with critical health needs. 





  • Vision and Hearing Care

    Make sure your sight and hearing are at their best. Benefits include:

    • Hearing.Tests, checkups and hearing aids, if needed, are provided.
    • Vision. Covers eye exams and eyewear.

  • Home Care and Supplies

    If you are recovering from a serious illness or surgery, you may need extra support. Our plan includes the care and equipment needed to recover safely at home. Benefits include:

    Nurse Visits. Care after surgery or illness to recover at home.

    Durable Medical Equipment. We cover wheelchairs, hospital beds and more.

  • Behavioral Health Services

    Get help with personal problems that may affect you or your family. These include stress, depression, anxiety, a gambling problem, or using drugs or alcohol.

  • Rides and Other Help

    Sometimes you might need a little extra help using your health plan. For those times, you can rely on:





    • Baby BlocksTM. Earn rewards for keeping you and your baby healthy.
    • Health Education. Free classes to help improve your health.
    • Language Help. Connect to a translator through Member Services.
    • Interpreters. Have an interpreter with you at your child’s doctor visits.
    • Member Advocate. Someone to get you information, services and care.
    • Transportation. Receive rides to and from approved medical visits.

  • Home and Community Services

    Our Long Term Support Services Plan is for adults and children who need extra support and care to help them live safely at home. You qualify based on income and health needs. If you are an adult or child living with a disability or illness, your health needs are unique. Our job is to make it as easy as possible for you to meet those needs and more.

    Benefits may include:




    • Adult Day Care. Friendship and social activities in a safe setting.
    • Care Coordinator. Someone to get you the services to be at your best.
    • Disease Management. Focused care for those with critical health needs.
    • In-Home Care. Medical and personal care at home, if needed.
    • Equipment and Supplies. In-home medical and safety equipment are covered.
    • Emergency Response System. Sends an urgent request for someone to help you.
    • Home Meal Delivery. If you cannot safely make meals, we’ll provide them.
    • Respite Care. In-home help to give your caregivers a rest. 




  • Adult Day Care

    Adult Day Care

    Companionship is important. Adult day care provides care and companionship for members who need extra help during the day. Caregivers also benefit, knowing their loved one is well cared for and safe during the day.

    Adult day care can provide:

    • Social connections and friendship.
    • Bathing and personal care.
    • Meals.

  • Asthma and COPD

    Asthma and COPD

    Do you or a family member have trouble managing asthma or COPD symptoms? A nurse who specializes in breathing issues can really help. You will get a customized treatment plan and medicine to:

    • Manage flare-ups.
    • Reduce symptoms.
    • Help you stay active.

  • Baby Blocks<sup>TM </sup>

    Baby BlocksTM

    Build a healthy future for you and your baby — and earn great rewards — through Baby Blocks. This program helps keep you and your baby healthy during your pregnancy and your baby’s first 15 months of life.

    Baby Blocks includes: 

    • Email appointment reminders.
    • Fun program to keep track of appointments.
    • Reward cards for clothing and more.

  • Care Coordinator

    Care Coordinator

    We provide a care coordinator to help members who need extra care. A care coordinator ensures the needed care and services are provided to help improve the member’s health. 
    The care coordinator:




    • Creates a care plan to match the member’s health care needs.
    • Coordinates care among the specialists and the primary care doctor (PCP).
    • Arranges for care and services after the member leaves the hospital. 




  • Care Management

    Care Management

    Do you have a disability or a serious health problem? If your health needs qualify then our care coordinators are in your corner. They will:





    • Explain medical terms in plain language. 
    • Coordinate your doctor appointments.
    • Provide your care team with your medical records.

    • Think beyond your immediate medical needs.
    • Make sure you have support at home.





  • Choice of Doctor

    Choice of Doctor

    You can choose your child's primary care physician (PCP). Use the Doctor Lookup tool to see if your doctor is in our network.

    The PCP is the main doctor for:

    • Preventive care.
    • Treatment if you are sick or injured.
    • Coordination of care with specialists for certain conditions.

    For members who don't have a doctor or whose doctor is not in our network, we can help find a new one close by.

  • Diabetic Support

    Diabetic Support

    If you have diabetes, we’ll help make life a little easier for you. You’ll get medicine, supplies and education to help you reach and stay your best.  

    Our diabetes program also includes self-care training and classes focused on:

    • Planning healthy meals.
    • Managing stress.
    • Living a healthy lifestyle.

  • Disease Management

    Disease Management

    If you have chronic or high-risk illnesses, our disease management services may help you. We provide support for:




    • Congestive heart failure.
    • Asthma.
    • HIV.
    • Chronic obstructive pulmonary disease (COPD).
    • Coronary artery disease.

    Your health and safety at home are important.

    Our plan covers medical equipment ordered by your doctor or care coordinator. This might include supplies like:





    • Oxygen tanks.
    • Hospital beds.
    • Wheelchairs or walkers.





  • Foot Care

    Foot Care

    Sometimes feet need extra attention. We provide the exams needed to keep feet in great shape. And for those with diabetes or a circulatory condition, good foot care helps prevent more serious problems.

    Our podiatry coverage includes:




    • Routine foot exams.
    • Special shoes and supports.
    • Information about nerve damage warning signs.

    Healthy First Steps®

    When you’re pregnant, you receive help and advice from your mom, aunts, sisters and girlfriends. Yet every pregnancy is different. This program will support you through your pregnancy.

    • Find a doctor for both mother and baby.
    • Get good advice on nutrition, fitness and safety.
    • Get supplies, including breast pumps for nursing moms.

  • Hearing Care

    Hearing Care

    Trouble hearing can affect your child's everyday life in many ways. Our plan includes services and support to help protect hearing.

    We cover:

    • Exams, aids and tests.
    • Hearing aids and supplies.

  • Home Meal Delivery

    Home Meal Delivery

    Good nutrition is essential to good health. If you live with a disability or chronic health condition, you may not be able to safely fix meals for yourself at home. So we’ll arrange for home meal delivery.

  • Hospitalization


    This plan pays for all expenses related to a hospital stay, so you can focus on helping your child get better.

    Hospitalization coverage includes:

    • Nursing care.
    • Room and board.
    • Supplies and equipment.
    • Treatment and therapies.
    • Diagnostic tests and exams.
    And after you leave the hospital, you are not alone. We make sure you get follow-up care to continue healing at home.

  • In Home Care

    In Home Care

    If you have health or mobility challenges, you may need extra help with day-to-day tasks. You may even need an in-home medical visit to check on how well you are healing. With approval, our plan covers:





    • In-home medical visits.
    • Medical equipment like hospital beds, walkers or wheelchairs.
    • Disposable medical supplies.
    • Help with personal care or housekeeping.






  • Interpreter Services

    Interpreter Services

    Your doctor and you need to understand each other. Not speaking English well makes this difficult. We can arrange for a medical interpreter to be at your appointments.

    We also have people in member services who speak more than one language. Chances are, we have someone who speaks your language.

  • Lab and X-rays

    Lab and X-rays

    Knowing what’s wrong and finding it early can make all the difference.  Our plan covers: 

    • Labs and testing.
    • X-rays, scans and other imaging.

    We’ll help you get the information needed to improve your health or be at your best. 

  • Language Help

    Language Help

    You can receive information in your preferred language. Just ask. We also have people at our customer service centers who speak more than one language. Chances are we have someone who speaks your language.

  • LTSS Services: Skilled Nursing Facility

    LTSS Services: Skilled Nursing Facility

    Services in skilled nursing facilities provide care for members needing help.  The benefit includes:




    • 24/7 nursing care.
    • Help with activates of daily living.
    • Services provided for long-term care needs.





  • Medicines


    Our plan includes prescription drugs and refills. There is a list of covered prescription drugs. Copays may apply.

    We also cover some over-the-counter medicines with a written order from your doctor. Copays may apply.

    And we make getting your medicine easy. You can fill your prescriptions at:

    • Local pharmacies in the UnitedHealthcare Community Plan network.
    • Services that deliver right to your door.

    Member Advocates

    We’re looking out for you. Member advocates can help solve problems, provide training and education, and work on issues that concern members.
    They can:

    • Provide information about your plan.
    • Teach you how to get the care you need.
    • Help with doctor issues or complaints.

    Mental health is as important as physical health. That’s why we have coverage for both.

    Medical questions and situations can come at inconvenient times. When you have questions about your child's health, you can call a nurse 24 hours a day, 7 days a week.

    Our nurses will:

    • Listen to symptoms.
    • Help with care questions.
    • Advise you about getting care for your child at a doctor visit or an urgent care center.
    • Help you know when to take your child to the emergency room.

  • Prenatal Visits

    Your pregnancy is a journey you’ll want to make with the help of friends, family and a pregnancy doctor, or OB-GYN.

    All recommended prenatal clinical visits and tests are covered by our plan.
    At these visits, the clinic will:



    • Make sure both you and your baby are healthy.
    • Explain what to expect at each stage of your pregnancy.
    • Answer your questions.




  • Respite Care

    Respite Care

    Do you have family members or friends who care for you at home? If so, we want to support their hard work too. That’s why we provide respite care to give your caregivers a rest. Respite care offers caregivers time away from their loved one who is ill or has special care needs.  Some members can get extra respite care. Ask your Care Coordinator if you qualify.

  • Text4baby


    Text4baby can help new and expecting mothers keep themselves and their babies healthy.

    You’ll get three text messages each week throughout your pregnancy and your baby’s first year. Each text contains health and safety tips for topics like:

    • Prenatal care.
    • Labor and delivery.
    • Breastfeeding.
    • Safe sleep.
    • Nutrition.
    •  Car Seat Safety.

    So you and your baby can make a great start on life.



  • Therapy Services

    Therapy Services

    Physical, occupational and speech therapy can help you recover from a serious injury or illness, or simply reach your full potential.

    Covered therapies include: 




    • Occupational therapy.
    • Physical therapy.
    • Speech therapy.




  • Transportation


    Our plan provides needed rides to and from health care locations. That includes trips to doctor or therapy visits.

  • Urgent Care

    Urgent Care

    You may have an injury or sudden illness that is not life-threatening but needs immediate attention. So our plan covers care at:

    • Urgent care centers.
    • Medical aid units.
    • Other walk-in clinics.

  • Vision Care

    Vision Care

    Members get vision exams and eyewear. Coverage includes $100 each year toward frames or contact lenses from a network vision care provider.

  • Well Visits

    Well Visits

    Well visits with your doctor are important for good health. These visits can catch health problems early, so they can be treated. Preventive services include:

    • Checkups for adults and children.
    • Well-baby care.
    • Care for women expecting a baby.
    • Routine shots and tests.
    • Mammograms. 
    • There are no copayments for preventive care.
