UnitedHealthcare Health & Home Connection


UnitedHealthcare Health & Home Connection is a coordinated care management program for individuals who are eligible for the Statewide Medicaid Managed Care Plan Long Term Care (SMMC LTC) Waiver Program. Our mission is to keep individuals living independently in their own homes for as long as possible.

UnitedHealthcare Health & Home Connection is administered by UnitedHealthcare of Florida, Inc. under a contract with the Agency for Health Care Administration. http://www.fdhc.state.fl.us/Medicaid/index.shtml

The Health & Home Connection SMMC LTC Program will be available in the following counties as of the start date listed for each Region:

2/1/2014 Region 6 Counties: 
Hardee, Highlands, Hillsborough, Manatee, and Polk

3/1/2014 Region 3 Counties: 
Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee, and Union

3/1/2014  Region 4 Counties: 
Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia

3/1/2014 Region 11 Counties: 
Miami-Dade and Monroe Counties

Eligibility for enrollment in the UnitedHealthcare Health and Home Connection plan is determined by CARES (Comprehensive Assessment and Review of Long-Term Care Services).
