Our M*Plus (MMA) plan covers children, families, pregnant women and single adults who may qualify based on income. UnitedHealthcare Health & Home Connection is a coordinated care management program for community-based elderly who are at risk of nursing home placement.

Alachua, Baker, Bradford, Citrus, Clay, Columbia, Dixie, Duval, Flagler, Gilchrist, Hamilton, Hardee, Hernando, Highlands, Hillsborough, Lafayette, Lake, Levy, Manatee, Marion, Miami-Dade, Monroe, Nassau, Polk, Putnam, St. Johns, Sumter, Suwannee, Union and Volusia.

You must send the complaint within 60 days of when you found out about it. A decision will be sent to you within 30 days. If you disagree with the decision, you have 15 days to ask us to look at it again.
