UnitedHealthcare Community Plan Medicaid(MMA)

This Florida Medicaid Plan offers a range of benefits. Children, families, pregnant women and single adults may qualify based on income. View the benefits below to see all that it offers.

We want to help you to get the most from your health plan. If you have not completed a health risk assessment, please call 888-716-8787 to talk with a representative from 8-5 pm M-F.

 

  • Prescription Medications and Medical Equipment: Members who are affected can also fill existing prescriptions early (one time, 90-day refill) and replace durable medical equipment, if necessary.  For mail-order delivery service to affected areas or any other questions related to their prescriptions, people are encouraged to call the pharmacy number on the back of their ID card, or speak directly to a pharmacist about their situation in order to get an early refill, if necessary.   

  • For Additional Assistance: Members can call the Customer Care number on the back of their medical ID card, if they need additional assistance.   If they have misplaced their medical ID card, they can call 866-633-2446, 8 a.m. – 8 p.m. (in the local time zone), Monday through Friday.

    People enrolled in employer-sponsored and individual health plans (excluding Medicare plans) who have a smartphone can download the free Health4Me app, which provides instant access to their ID card, contact information for network care providers, their personal health benefits and more. The Health4Me app is available as a free download at the Apple iTunes App Store  and the Android Market on Google Play.

These measures apply to all individuals insured by UnitedHealthcare and are effective through October 21, 2018 and will be reviewed again at that time if necessary.  If you have questions related to coverage, please call Customer Care at the number on the back of your ID card.


Help For Broader Community:

  • Free Help Line: Optum is offering a free emotional-support help line to affected individuals. The toll-free number, 866-342-6892, will be open 24 hours a day, seven days a week, for as long as necessary. The service is free of charge and open to anyone.  Callers may also receive referrals to community resources.  Along with the toll-free help line, emotional-support resources and information are available online at www.liveandworkwell.com.

  • Shots and Vaccines. Routine shots can help keep you or your child healthy.

Care for Conditions

If you have asthma, diabetes or another long-term condition, you can depend on us. Our plan makes sure you get the care and services you need. Benefits include:

  • Mental Health. Counseling and other treatments are covered.
  • Care Management. Personal support from an experienced professional.
  • Asthma and Allergy Care. Exams, testing and supplies are covered.
  • Diabetes Support. Needles, supplies, plus classes on managing diabetes.
  • Residential Care. Brief stay at a care facility while you recover from sickness or injury.

Vision, Dental and Hearing Care

Make sure your sight, smile and hearing are at their best. Benefits include:

  • Vision. Eye exams and up to two pairs of eyeglasses each year.
  • Dental. We cover cleanings, checkups and dental work.
  • Hearing. Tests, checkups and hearing aids, if needed, are provided every three years.

    Attention Diabetic Members – As of March 2015, referrals from your Primary Care Physician are no longer needed for your annual Diabetic Retinal Eye Exams with an Optometrist. Referrals are still required to see an Ophthalmologist.

Home Care and Supplies

Get the medical care and equipment you need to live safely at home. For example, your care provider might recommend:

  • Nurse Visits. Medical care to help you safely recover at home.
  • Equipment and Supplies. In-home medical and safety equipment is covered.

Rides and Other Help

Sometimes you might need a little extra help using your health plan. For those times, you can rely on:

  • NurseLineSM. Speak with a registered nurse 24/7.
  • Interpreters. An interpreter can go with you to your medical visits.
  • Member Services. Your benefit questions are answered 24/7.
  • Transportation. We cover unlimited rides to and from provider offices.
  • Quitting Tobacco. Coaches and supplies to help you quit tobacco.
  • Over-the-Counter Supplies. Get up to $25 back on approved over-the-counter purchases.
