Special Needs BasicCare (SNBC) Medicaid Only Plan - Lookup Tools

Primary Care Provider

When you are a member or become a member of UnitedHealthcare you chose or were assigned to a primary care provider (PCP). Your primary care provider (PCP) can provide most of the health care services you need and will help coordinate your care. You may change your primary care provider (PCP). Contact Member Services to change your provider.

You do not need a referral to go to a Plan network specialist. However, your primary care clinic can provide most of the health care services you need and will help coordinate your care.

Contact your primary care clinic for information about the clinic’s hours prior authorizations, and to make an appointment. If you cannot go to your appointment, call your clinic right away.

You may change your Primary Care Provider or clinic. To find out how to do this, call Member Services at 1-888-269-5410, TTY 711.

Emergency and Urgent Care

The Emergency and Urgent Care List tells you what a real emergency is and provides a list of many urgent care centers that you can use. You can use these centers when it’s not a life-threatening situation, but you are sick or need medical care right away and your PCP is not available. Urgent care centers are usually open in the evenings and on weekends.

Emergency rooms (ERs) are for major injuries or sudden sicknesses that may be life threatening and need medical care right away. In an emergency, you should call 911 or go to an ER immediately. Your member handbook includes details about how to get approval to visit the ER.
