Information about COVID-19 is available from the Centers for Disease Control (CDC) and the Arizona Department of Health Services. The Arizona Poison Control System is now available to take COVID-19 calls from Arizona providers and the general public at 1-844-542-8201.

AHCCCS members can schedule non-emergency transportation, if needed, to attend drive-through and other vaccine appointments. Please call at least 72 hours before your appointment. Call 1-888-700-6822 (option 3) or 602-889-1777 (option 3), TTY/TDD 711 to set up your ride.

This dual health plan is for people who qualify for both Medicaid and Medicare Parts A & B (Original Medicare).

This is a Health Maintenance Organization (HMO) plan. With an HMO plan, you must use network providers to get your medical care and services. 

For those receiving AHCCCS Complete Care (ACC) benefits, this plan is available in the following counties: Gila, Maricopa, Pima, Pinal

For those receiving ALTCS-DD benefits, this plan is available in the following counties: Apache, Cochise, Coconino, Graham, Greenlee, La Paz, Maricopa, Mohave, Navajo, Pima, Pinal, Santa Cruz, Yavapai, Yuma

This plan is available in all counties.

This dual health plan is for people who qualify for both Medicaid and Medicare Parts A & B (Original Medicare).

This is a Health Maintenance Organization (HMO) plan. With an HMO plan, you must use network providers to get your medical care and services. 

This plan is a Fully Integrated Dual Eligible Special Needs Plan (FIDE-SNP) for people who have both Medicare A and B, and full Medicaid Long Term Care benefits. You must live in our service area and be enrolled in an AHCCCS Long Term Care Plan.

Apache, Coconino, Gila, Maricopa, Mohave, Navajo, Pinal, and Yavapai.

AHCCCS Complete Care


AHCCCS Complete Care is an integrated health insurance plan for Arizona residents who meet certain income and other requirements. It offers physical and behavioral health services together to treat all aspects of your health care needs including doctor visits, hospitalization, prescription drugs and services specific to CRS conditions.

AHCCCS Complete Care Members with Children's Rehabilitative Services (CRS) Designation. Some AHCCCS Complete Care Members have been diagnosed with a CRS condition and are designated as CRS. Members who are designated CRS may receive specialized care.

You can choose your own doctor and get extra programs that only UnitedHealthcare Community Plan provides.

Developmental Disabilities Program


Our Arizona Developmental Disabilities (DD) Program assists with the behavioral health and physical health needs of children and adults with conditions such as autism spectrum disorder, epilepsy,, cerebral palsy or other cognitive disabilities. Members can choose their own care providers. And they get many services to help the family manage their care.

This plan is available in all counties.

Long Term Care


Our Arizona Long-Term Care program is for people of all ages who have a long-term disability due to age, illness or injury. 

You can choose your own doctor. And you get member empowerment services to help with daily activities that  only UnitedHealthcare Community Plan offers.

KidsCare


Health insurance is a benefit every Arizona family should have – especially for children. Our Arizona KidsCare Children’s Health Insurance Program (CHIP) is for children under age 19 who are not eligible for other AHCCCS health insurance. You can choose your own doctor and get extra programs that only UnitedHealthcare Community Plan provides.
