If your children ages 0 to 13 are Community Plan members, they can earn points for healthy habits, like brushing teeth and eating healthy.

As the parent or guardian, you  also earn points for the things you do to help your child stay healthy, like taking your child to doctor visits and reading our member newsletter.

Non-emergency transportation is offered through the State of Wisconsin and is not covered by UnitedHealthcare.

Call the UnitedHealthcare Customer Service Department at 1-800-504-9660 and say "transportation". You will be transferred directly to the state transportation vendor.

UnitedHealthcare provides ambulance service for emergency care. Non-emergency trips are covered when arranged by your provider.

You may also need a pneumonia shot if:

  • Your child’s doctor advises it.
  • Your child has long-term health problems like asthma or a weakened immune system.
