UnitedHealthcare Community Plan (ABD)

If you are eligible for Ohio Medicaid, you and/or members of your family may receive health care benefits through either the Covered Families and Children (CFC) program (including Healthy Start and Healthy Families) or the Aged, Blind, or Disabled (ABD) program. CFC Medicaid consumers include families, children up to age 19, and pregnant women. ABD Medicaid consumers include adults age 65 and older and Ohioans who are blind or disabled, over the age of 21. On our website, you will see some information marked either for CFC or ABD. Some information may be different, depending on which Ohio Medicaid program you receive. UnitedHealthcare Community Plan brings you more than just great basic health coverage. This plan provides extra services that help make your life easier such as thousands of network providers, $0 copays, a 24/7 NurseLine to answer your health questions and free transportation to your medical appointments. UnitedHealthcare Community Plan is a plan for Ohio Medicaid consumers who are aged, blind or disabled, over the age of 21. Certain exclusions apply.

Children with Special Health Care Needs*
If you have a child who is currently eligible for Medicaid due to a disability and receives a monthly medical card, this is the plan for you. The transition from fee-for-service Medicaid to Ohio's Managed Care program means UnitedHealthcare Community Plan is now available for children with special health care needs all across Ohio.

Enroll now for service beginning July 1, 2013.

UnitedHealthcare Dual Complete™ (HMO SNP)

This is a Medicare Advantage plan for people on Medicaid who also qualify for Medicare – commonly known as "dual eligible." It's intended for people who live in a community setting, receive financial help (such as SSI) from their State Medicaid program and have Medicare. This plan includes all Medicare Part A (Hospital) & B (Doctor) benefits and Part D prescription drug coverage. It also provides benefits not offered by Original Medicare.

This plan is available in the following counties: Butler, Clark, Cuyahoga, Franklin, Greene, Hamilton, Madison, Mahoning, Montgomery, Stark, Summit, Trumbull and Warren.
