Medicaid SSI and UnitedHealthcare Dental

Financial hardships or disability can make life difficult in many ways – but having a capable health care plan can help. Medicaid SSI is a program for individuals 19 years and older who receive Medicaid SSI or SSI-related Medicaid because of a disability determined by the Disability Determination Bureau.

What is Supplemental Security Income (SSI)?

Supplemental Security Income (SSI) is a monthly cash benefit paid by the federal Social Security Administration (SSA) and state Department of Health and Services (DHS) to low-income elderly, blind, and disabled residents of Wisconsin.

UnitedHealthcare Dental

If you live outside of Milwaukee, Racine, Kenosha or Waukesha counties, you may get dental services from any dentist who will accept your ForwardHealth ID card. Your dental services are provided by the State, not UnitedHealthcare. Call our Customer Service Department at 1-800-504-9660 for the names of our dentists. You may also view our UnitedHealthcare Dental Provider Directory under the "Find a Provider" tab on our website.

This plan is available in the following counties: Ashland, Bayfield, Brown, Burnett, Calumet, Dodge, Door, Douglas, Fond du Lac, Forest, Green Lake, Jefferson, Kenosha, Kewaunee, Langlade, Manitowoc, Marathon, Marinette, Marquette, Menominee, Milwaukee, Oconto, Oneida, Outagamie, Ozaukee, Pierce, Polk, Portage, Price, Racine, Rock, Sawyer, Shawano, Sheboygan, St. Croix, Taylor, Vilas, Walworth, Washburn, Washington, Waukesha, Waupaca, Waushara, Winnebago and Wood.
