This plan features $0 copays. It also provides outstanding vision care, free transportation to medical appointments and a 24/7 NurseLine to answer your health questions.

This plan is available in the following counties: Ashland, Athens, Belmont, Carroll, Columbiana, Coshocton, Gallia, Guernsey, Harrison, Holmes, Jackson, Jefferson, Lawrence, Mahoning, Meigs, Monroe, Morgan, Muskingum, Noble, Portage, Richland, Stark, Summit, Trumbull, Tuscarawas, Vinton, Washington and Wayne.
