UnitedHealthcare Community Plan is a Health Maintenance Organization (HMO). We offer high quality health care services to those eligible for Michigan Medicaid.

With our plan, you have a personal doctor to work with for all your FREE health care needs. Your personal doctor is your Primary Care Provider (PCP). You get most of your care from your PCP. Your PCP will give you the care you need or send you to a UnitedHealthcare Community Plan provider or specialist. UnitedHealthcare Community Plan providers or specialists are doctors, hospitals, labs, pharmacies and other medical professionals. They work together to make sure you get the best care.

There are many health care programs for children, adults, and families. The Michigan Department of Human Services (DHS) can tell you which program(s) you can join. It will depend on your income and other factors.

This plan is available in the following counties: Allegan, Berrien, Branch, Calhoun, Cass, Hillsdale, Huron, Jackson, Kalamazoo, Kent, Lenawee, Livingston, Macomb, Monroe, Muskegon, Oakland, Oceana, Ottawa, Saginaw, Sanilac, St. Clair, St. Joseph, Tuscola, Van Buren and Wayne.
