UnitedHealthcare Community Plan

There are many health care programs for children, adults, and families. The Michigan Department of Human Services (DHS) can tell you which program(s) you can join. It will depend on your income and other factors. To find a DHS office near you go to www.michigan.gov/dhs.

If you have Medicaid now and want to join UnitedHealthcare Community Plan:

You can join our plan if you live in: Allegan, Berrien, Branch, Calhoun, Cass, Hillsdale, Huron, Jackson, Kalamazoo, Kent, Lenawee, Livingston, Macomb, Monroe, Muskegon, Oakland, Oceana, Ottawa, Saginaw, St. Clair, St. Joseph, Sanilac, Tuscola, Van Buren or Wayne County.
