UnitedHealthcare Community Plan M*Plus

You must have Medicaid in order to be part of this health Plan. Most Medicaid recipients have to choose to be in a Medicaid HMO or MediPass. If you do not choose an HMO or MediPass, the State of Florida will choose one for you. If you did not request to enroll with UnitedHealthcare of Florida (Medicaid), then the State of Florida has placed you in this health Plan. We have picked a primary care doctor for you, and your provider's name is on your ID card. You may choose to change your primary care doctor and can call Member Service to help you do this. Also, you may choose to have all family members, who are on UnitedHealthcare Community Plan, served by the same primary care doctor, or you may choose different primary care doctors based on each person's needs.

You can provide feedback directly to Medicare through their Complaint Form about your Medicare health plan.
