To become a member of Unison Health Plan, you must first apply for the D.C. Healthy Families program. Please visit the D.C. Healthy Families website.

D.C. Healthy Families is a free health insurance program funded by the District of Columbia and the Federal Government. It is for families with children under the age of 19 and for women who are pregnant that live in the District of Columbia.

If you do not qualify for a Medicaid program, you may be eligible for the D.C. Health Care Alliance program. If you are eligible, you can choose Unison as your plan. Alliance is a public healthcare program for individuals and families who meet the following requirements:

• Live in the District of Columbia
• Have no health insurance
• Have a pre-taxed family income equal to or below 200% of the Federal Poverty Level.

Your eligibility depends on family income guidelines set by the Department of Health.
