Arizona Physicians IPA Children's Rehabilitative Services

UnitedHealthcare Arizona Physicians IPA Children's Rehabilitative Services does not determine eligibility based on income. To be eligible for UnitedHealthcare Arizona Physicians IPA Children's Rehabilitative Services services you must:

Have certain medical conditions,
Be under age 21,
Be an U.S. citizen or qualified alien, and
Live in Arizona.

To be enrolled in UnitedHealthcare Arizona Physicians IPA Children's Rehabilitative Services, you must also be enrolled in an AHCCCS or an ALTCS plan. If you lose your AHCCCS or ALTCS enrollment for any reason, you will be disenrolled from CRS. Make sure you keep current with AHCCCS or ALTCS. Respond to any request from AHCCCS or ALTCS to update your coverage.

You can provide feedback directly to Medicare through their Complaint Form about your Medicare health plan.
