Evercare Select® is a long-term-care health plan for certain elderly, physically disabled and chronically ill people who are recipients of Medicaid. It is offered through Evercare of Arizona, Inc. Services are funded, in part, through a contract with the state of Arizona. In order to be a member of Evercare Select®, you must be enrolled in the Arizona Long Term Care System, which is part of the Arizona Health Care Cost Containment System (AHCCCS).

Evercare Select draws upon strong local and national experience in serving people needing long-term care. Our extensive network of physicians, nursing facilities, assisted living facilities and other care services meet the special and changing needs of individuals. Our case managers are specifically trained in understanding the unique and comprehensive needs of individuals who require coordinated long-term care services.

Our case managers get to know you as a person and work directly with you and your family members to coordinate and personalize the services you need. Whether you live in your own home, an adult foster care home, an assisted living home, an assisted living center or a nursing facility, Evercare Select can help you get the resources you need to maintain your health and well-being.

Effective October 1, 2011 Evercare Select will also be available in LaPaz, Pima, Santa Cruz, Yavapai, and Yuma Counties.

This plan is available in the following counties: Apache, Coconino, La Paz, Maricopa, Mohave, Navajo, Pima, Santa Cruz, Yavapai and Yuma.
