When you're arranging for the care of a loved one, finding your way through the maze of doctors, assisted living homes and nursing facilities can be confusing and overwhelming. At Evercare, we understand the importance of the decisions you have to make. Our purpose, as care advocates for the elderly and physically disabled, is to understand the level of care that is needed. We then meet those needs with specific services that are designed to enhance an individual's quality of life, while maintaining his or her dignity and independence.
Personalized Care
Our Evercare Select Case Managers are skilled professionals who work with individuals enrolled in the Arizona Long Term Care System (ALTCS) and their families or guardians who are involved in that person's care. We hire experienced and professional care managers who are committed to spending time with you. A personal case manager meets with you within 12 business days of enrolling in Evercare Select. By getting to know you personally, we may be able to detect a problem before it turns serious. We work together to develop a thorough, personalized plan of care that not only addresses medical needs, but also social, emotional and environmental services. Our case managers stay involved through regular visits and conferences with each individual and his or her family or guardian. Our team-oriented approach, centered on the needs and wishes of the individual, helps ensure that quality care is provided in a setting that maximizes comfort and independence.
Evercare can arrange multiple services as needed, such as:

- Housekeeping services
- Meal preparation assistance
- Home medications
- Attendant care
- Pharmacies
- Personal care
- Home-delivered meals
- Daily medication monitoring
- Home health aides
- Respite care (relief for caregivers)

An individual's unique personalized care is delivered in a variety of settings. These include:
Home and Community Based Settings (HCBS)
Medical, social and supportive services are provided to people living in their own home, apartment or assisted living facilities, including adult foster care and supportive residential units.
Nursing Facility Settings
Individuals choose from approximately 90 nursing facilities located throughout the state of Arizona. Evercare will also work closely with Medicare Advantage Plans or other insurance companies to coordinate all the services required.
