Every three months you'll receive $50 in credits you can use to order health products. The products will be delivered directly to you at no additional cost.

  • Thermometers, blood pressure monitors and more.
Transportation Assistance

Transportation Assistance

Up to 24 one-way rides every year.

We can help you get to plan-sponsored locations so you can take care of your health needs. 24 one-way trips are available at no additional cost to you.

Transportation coverage includes:

  • Rides to health providers like doctors and dentists.
  • Rides to your pharmacy.
  • Handicap accessible vehicles as needed.
Eyewear Coverage

Eyewear Coverage

$0 copay on eye exams and get credit toward eyewear.

Vision care can help protect your eyes and can also be an important part of your overall health. There is no additional cost to you.

Vision coverage includes:

  • One covered eye exam every 2 years.
  • $70 credit on glasses or $105 credit on contacts every 2 years.

UnitedHealthcare Dual Complete Plans

Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract and a contract with the state Medicaid program. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is available to anyone who has both Medical Assistance from the State and Medicare. You must continue to pay your Medicare Part B premium, if not otherwise paid for under Medicaid or by another third party. This information is not a complete description of benefits. Contact the plan for more information. Limitations, co-payments, and restrictions may apply. Benefits, premiums and/or co-payments/co-insurance may change on January 1 of each year. UnitedHealthcare Senior Care Options (HMO SNP) Plan

UnitedHealthcare SCO is a Coordinated Care plan with a Medicare contract and a contract with the Commonwealth of Massachusetts Medicaid program. Enrollment in the plan depends on the plan’s contract renewal with Medicare. This plan is a voluntary program that is available to anyone 65 and older who qualifies for MassHealth Standard and Original Medicare. If you have MassHealth Standard, but you do not qualify for Original Medicare, you may still be eligible to enroll in our MassHealth Senior Care Option plan and receive all of your MassHealth benefits through our SCO program.
