UnitedHealthcare Community Plan (ABD) brings select Ohio residents more than just great basic health coverage. This plan provides extra services that help make your life easier such as thousands of network providers, $0 copays and free transportation to your medical appointments. UnitedHealthcare Community Plan is a plan for Ohio Medicaid consumers who are aged, blind or disabled, over the age of 21. Certain exclusions apply.

Children with Special Health Care Needs*
If you have a child who is currently eligible for Medicaid due to a disability and receives a monthly medical card, this is the plan for you. The transition from fee-for-service Medicaid to Ohio's Managed Care program means UnitedHealthcare Community Plan is now available for children with special health care needs all across Ohio.

To enroll in this health care program, call Ohio Department of Job and Family Services: 1-800-324-8680.

*Aged, Blind or Disabled (ABD) children (Please note: this transition will not affect children eligible for Medicaid on waivers; or children who reside in institutional settings; or children who receive both Medicare and Medicaid benefits.)
