The Medicare Advantage plans we offer are for people who qualify for both Medicaid and Medicare Parts A and B. Our plans give you more benefits and features than Original Medicare. And you still receive all of your Medicaid benefits.
If you are eligible for Ohio Medicaid, you and/or members of your family may receive health care benefits through either the Covered Families and Children (CFC) program (including Healthy Start and Healthy Families) or the Aged, Blind, or Disabled (ABD) program. CFC Medicaid consumers include families, children up to age 19, and pregnant women. ABD Medicaid consumers include adults age 65 and older and Ohioans who are blind or disabled at any age. On our website, you will see some information marked either for CFC or ABD. Some information may be different, depending on which Ohio Medicaid program you receive. If you have any questions, or you are not sure if you are a CFC or ABD member, please call UnitedHealthcare Community Plan at 1-800-895-2017 (TTY# 711) and we will be happy to help you.
Families and children in Ohio have a great option for health care coverage. UnitedHealthcare Community Plan offers qualifying low-income families, children up to age 19, and pregnant women, more than just basic coverage. This plan features $0 copays. It also provides outstanding vision care, free transportation to medical appointments and a 24/7 NurseLine to answer your health questions. UnitedHealthcare Community Plan is a plan that Covered Families and Children Medicaid consumers, including Healthy Start and Healthy Families can join. Certain exclusions apply.
Children with Special Health Care Needs* If you have a child who is currently eligible for Medicaid due to a disability and receives a monthly medical card, this is the plan for you. The transition from fee-for-service Medicaid to Ohio's Managed Care program means UnitedHealthcare Community Plan is now available for children with special health care needs all across Ohio.
To enroll in this health care program, call the Ohio Department of Job and Family Services: 1-800-324-8680.
*Aged, Blind or Disabled (ABD) children (Please note: this transition will not affect children eligible for Medicaid on waivers; or children who reside in institutional settings; or children who receive both Medicare and Medicaid benefits.)
