How To Enroll

After December 31, 2013, you can no longer apply for Family Health Plus. If you think you may qualify, you can apply on the New York State of Health website, or by calling 1-855-355-5777.

You may be eligible to join if you live in any of the following counties: Albany, Bronx, Brooklyn, Broome, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Essex, Fulton, Genesee, Herkimer, Jefferson, Madison, Monroe, Nassau, New York (Manhattan) Niagara, Oneida, Onondaga, Orange, Oswego, Queens, Rensselaer, Rockland, Staten Island, St. Lawrence, Tioga, Ulster, Warren and Westchester.

UnitedHealthcare Community Plan has Certified Application Counselors in your neighborhood to help you complete your application on the New York State of Health website. Just call 1-888-617-8979 and a representative will call you to schedule an appointment.
