With UnitedHealthcare Community Plan, children may be eligible for disease management services if they have serious injuries or chronic or high-risk illnesses. We cover:

  • Diabetes
  • Congestive Heart Failure
  • Asthma
  • HIV
  • Chronic Obstructive Pulmonary Disease
  • Coronary Artery Disease

As a member, you can call and talk to a skilled nurse anytime, day or night. Call our NurseLine with questions about:

  • Your child’s symptoms.
  • How to care for your sick child.
  • What to do if your child is hurt.
  • If you should take your child to the doctor, urgent care or the emergency room.

Members, call 1-800-850-1267 (TTY:711) or call 1-855-634-3571 to talk to a doctor. If you are not a member, select a plan from the list on the left.
