Well and Sick Care

Get the care you need to be at your best — or to get better if you are injured or sick. Member benefits include:

 

  • Care Coordinator. Someone to get you the services to recover safely.
  • Case Management. If you qualify, get personal help managing health conditions.
  • Chiropractor Services. We pay for up to <XX> chiropractic visits per year. 
  • Choice of Doctor. Find a doctor you trust in our network.

  • Hospitalization. You pay nothing for care in a hospital.

  • Transportation. We pay for rides to and from medical visits.
  • Vision Care.Covers eye exams and glasses every two years.
  • Well Visits.Annual checkups to help keep you healthy.
  • Lab and X-rays. Lab tests, x-rays and diagnostic imaging are covered.
  • Urgent Care.Convenient care for a sudden illness or injury.
  • Speech. Screenings and needed therapies are provided

  • Asthma and COPD Care. Exams, testing and supplies are covered.
  • Care Coordinator. Someone to get you the services you need to recover safely.
  • Care Management. If you qualify, get personal help managing health conditions.
  • Diabetic Support. Medicine, needles and wipes for managing diabetes.
  • Disease Management. Focused care for those with critical health needs.
  • Kidney Disease Care. Dialysis, medicine and treatments are covered.
  • Mental Health. Counseling and other treatments are covered.
  • Obesity Care. We cover screenings and counseling for obesity.
  • Residential Care. Brief stay at a care facility while you recover from sickness or injury.
  • Special Needs Unit. Special help getting care and services for challenging health needs.
  • Heart Care.Extra help and support for members who have coronary artery disease.

    Vision, Dental, Speech and Hearing Care

    Make sure your sight, smile and hearing are at their best. Benefits include:

     

  • Dental.We cover cleanings, checkups and dental work.
  • Hearing.Tests, checkups and hearing aids, if needed, are provided.
  • Vision. Eye exams and glasses covered to age 21.
  • Speech. Screenings and needed therapies are provided.
  • Home Care and Supplies

    If you are recovering from a serious illness or surgery, you may need extra support. Our plan includes the care and equipment needed to recover safely at home. Benefits include:

     

  • Adult Day Care. Friendship and social activities in a safe setting.
  • Care Coordinator. Someone to get you the services you need to recover safely.
  • Caregiver Training. Education and support for those who care for others.
  • Chore Services. Someone to do your errands, cleaning or laundry.
  • Personal Care. Someone to help with dressing, bathing and dining.
  • Emergency Response System. Sends an urgent request for someone to help you.
  • Equipment and Supplies. In-home medical and safety equipment are covered.
  • Durable Medical Equipment. We cover wheelchairs, hospital beds and more.
  • Home Meal Delivery. If you can’t safely make meals, we’ll provide them.
  • Nurse Visits. Care after surgery or illness to recover at home.
  • Respite Care. In-home help to give your caregivers a rest.
  • In-Home Care. Medical and personal care at home, if needed.
  • Home Health Care. Medical and personal care in your home, if needed.
