We are pleased to introduce UnitedHealthcare Connected – which will be available as of July 1, 2014, to qualified individuals who are eligible for both Medicare and Medicaid in: Bronx, Kings, New York, Queens and Nassau counties.
UnitedHealthcare Connected is a partner in the New York State Department of Health’s Fully Integrated Duals Advantage (FIDA) program for Medicare-Medicaid eligible enrollees. UnitedHealthcare Connected is dedicated to help improve quality and efficiency of care benefits for members through an individualized approach that focuses on seamless coordination of care while simultaneously easing the administrative burden for providers through assistance with higher acuity patients and claims simplification.
