Long Term Care

Our Long Term Care Plan is people of all ages who have a long-term disability due to age, illness or injury. Our goal is to offer services in a community setting instead of nursing home placement when possible. You get member empowerment services to help with daily activities that only UnitedHealthcare Community Plan offers. You qualify based on income and health needs. Well and Sick Care

Get the care you need to be at your best — or to get better if you are injured or sick. That includes:

  • Choice of Doctor. Find a doctor you trust in our network. 
  • Medicines. Fill your prescriptions at local network pharmacies. 
  • Transportation. We pay for rides to and from medical visits or the pharmacy.
  • Case Manager. Someone to help set up and schedule your care,
  • NurseLineSM. Speak with a registered nurse 24/7.

Care for Conditions

If you have asthma, diabetes or another long-term condition, you can depend on us. Our plan makes sure you get the care, support and services you need. Benefits include:

  • Mental Health. Counseling and other treatments are covered.
  • Case Management. Our case managers work directly with you and your family to coordinate and personalie the services you need. 
  • Asthma and COPD Support. Individual care to help you control asthma or COPD.

 

Home Care and Supplies

If you are recovering from a serious illness or surgery, you’ll need extra support. Our plan includes the care and equipment needed to recover safely at home. Benefits include:

  • Personal Care. Services to aid healing after illness or injury.
  • Home Meal Delivery. If you can’t safely make meals, we’ll provide them.
  • Equipment and Supplies. In-home medical and safety equipment are covered.
  • Nurse Visits. Care after surgery or illness to recover at home.
  • Respite Care. In-home help to give your caregiver a rest. 

Rides and Other Help

Sometimes you might need a little extra help using your health plan. For those times, you can rely on:

  • Transportation. We pay for rides to and from medical visits or the pharmacy.
  • NurseLineSM. Speak with a registered nurse 24/7.
  • Interpreters. An interpreter can go with you to medical visits.
  • Language Help. Translated medical materials are available at no cost. 
  • Social Services. We’ll help connect you to community services.
  • Member Services. Your benefit questions are Monday - Friday (8 a.m. - 5 p.m. Local time).
